About the Role:

Grace at Work is looking for a compassionate and organized Intake Coordinator to join their team in Bedford, IN. Responsibilities:

Manage the client intake process from initial inquiry through enrollment and onboarding

Conduct intake screenings, assessments, and interviews with prospective clients and families

Maintain accurate and up-to-date client records, documentation, and intake forms

Coordinate with internal care teams to ensure smooth client transitions and service placement

Respond promptly to phone, email, and in-person inquiries with professionalism and empathy

Track intake data and generate reports to support organizational goals

Ensure compliance with applicable regulations, policies, and confidentiality standards

Requirements:

Prior experience in intake coordination, case management, social services, or a related field preferred

Strong interpersonal and communication skills with a compassionate, client-first approach

Proficiency in data entry, electronic health records (EHR), or case management software

Ability to multitask, prioritize, and stay organized in a fast-paced environment

High school diploma or GED required; associate's or bachelor's degree in human services or related field a plus

Knowledge of HIPAA and client confidentiality requirements

Reliable, detail-oriented, and committed to serving vulnerable populations
