Position Overview Trinity Healthcare Talent Services is seeking a Travel Physical Therapist (PT) for a home health assignment in Valley City, ND . This role involves providing therapy services to patients in their homes across a 60‑mile radius , treating a mix of post‑surgical, fall recovery, and general home health cases. Key Responsibilities Conduct in‑home patient evaluations and develop care plans Provide skilled PT treatment to home health and hospice patients Manage a caseload of 5–7 patients per day Educate patients and families on mobility, safety, and recovery Coordinate care with interdisciplinary teams Requirements Active North Dakota PT License Minimum 1 year of Home Health experience BLS Certification (AHA) Valid Driver's License & Auto Insurance Strong time management and independent work skills Schedule: Monday–Friday, 8:00 AM – 5:00 PM 40 hours guaranteed Coverage area: ~60‑mile radius Must live 50+ miles from facility Cannot have worked with CommonSpirit/CHI/Dignity within the past 12 months Facility may cancel up to 4 shifts per contract #J-18808-Ljbffr Trinity Healthcare Talent Services, LLC
