Common diagnosis / Types of patients: Home health clients - surgeries, falls, etc.

Acute Inpatient, Acute Rehab, Skilled, Outpatient?: Home Health and Hospice patients

Areas required to cover: 60mi radius around Valley City, ND

EMR: HomeCare/HomeBase

PT Manager/Supervisor on each shift?: No

Staff Rehab Aides: PTAs?: This is a possibility

Shift Details: Monday - Friday 8a-5p

Scrub Color/Dress Code: CSH color scrub or Business Casual

Number of shifts of unit/department specific orientation: 3-5

Radius Rule: 50 miles AND candidates may not be full time, part time, or PRN with any facility associated with CommonSpirit, including CHI and/or Dignity within the past 12 months. RTO needed upfront - Additional RTO will not be approved, including after submissions.
