Physical Therapist PT Home Care

Job Overview Primary City/State: Casa Grande, Arizona Department Name: Home Care Team-East Work Shift: Day Job Category: Clinical Care Up to $10,000 sign‑on bonus if eligible. Schedule Training Core: Monday‑Friday 8:00am‑5:00pm for the first 30‑60 days. After training, you may choose from the following options: Option 1 – Full Time (32‑40hrs per week): Monday‑Friday, 410‑hr shifts or 58‑hr shifts, with weekend work optional. Option 2 – Part Time (20‑28hrs per week): Monday‑Friday, 58‑10‑hr days, plus one weekend day. Option 3 – BSS/PRN (as needed): Minimum two shifts per month. Commitment for full‑time: working every other weekend with weekdays on off weeks or working every weekend with flexibility of one weekday. Locations East Valley – POD 1: East Mesa, Gold Canyon, Apache Junction, Superior. East Valley – POD 2: Mesa, Gilbert, San Tan Valley, Queen Creek, Florence. East Valley – POD 3: NE Mesa. East Valley – POD 6: Casa Grande, Arizona City, Eloy, Coolidge, Maricopa. East Valley – POD 8: Phoenix, Mesa, Tempe, Chandler. East Valley – POD 9: Phoenix, Scottsdale, Fountain Hills, Cave Creek, Carefree. POSITION SUMMARY This position is responsible for the evaluation, planning, administration and supervision of Physical Therapy treatment in accordance with physician referral, state and national organizational standards, regulatory agencies, and established departmental policies and procedures. The role demonstrates clinical competence and knowledge necessary to provide treatment appropriate to the age of the patients served. CORE FUNCTIONS Evaluates patient’s functional abilities and limitations; determines need for intervention; establishes a plan of care and treatment goals; completes comprehensive evaluation within the established time frame. Identifies and prioritizes key limiting factors; establishes functional, measurable, patient‑related treatment goals reflecting those limiting factors. Collaboration with patient and family/caregiver when setting goals; initiates discharge planning; recommends additions or modifications to referring orders. Implements individualized treatment plan considering the patient’s physical, social, spiritual, cultural, educational, and age‑specific needs; provides skilled treatment safely. Assesses ongoing effectiveness of treatment and modifies plan of care as indicated; involves patient and family/caregiver in treatment and decision‑making. Provides ongoing education to physician, nurse, patient, family/caregiver, and interdisciplinary team. Documents according to professional practice guidelines set by regulatory agencies; documents legibly; completes documentation within the established time frames of the department; documents evidence‑based and individualized treatment techniques/plan. Communicates pertinent patient information to ensure best possible follow‑through of care; communicates evaluation, treatment results and patient progress to physicians, nurses, patient, family/caregiver, and interdisciplinary team. Accepts responsibility to contact physicians for routine or difficult problem‑solving to ensure optimal patient outcome; defends treatment rationale with standard practices and/or current literature. Exhibits adaptability in assignments as it relates to caseload, scheduling, and staffing shifts; takes initiative to assist in other patient care areas and accepts responsibility for all patient care needs. Demonstrates appropriate supervision and delegation of personnel to achieve safe delivery of quality cost‑effective patient care; supervises and delegates to PTA, ATC, Therapy Technician, and student in accordance with department policies and regulatory agencies. Participates in training and orientation of new personnel; assures competency of personnel prior to delegation of tasks. Attends departmental required meetings and follows up to obtain pertinent information; participates in departmental/facility process improvement; meets productivity standards set by the department. Interacts with all levels of staff in a variety of departments, physicians, patients, families, and external contacts such as employees of other health care institutions and community agencies concerning the health care of the patient. Also interacts with physician offices to report, request or clarify information. MINIMUM QUALIFICATIONS Graduate from an APTA accredited school of Physical Therapy, or equivalent. Current Physical Therapy license within the State of practice. (Internationally trained candidates have six months from the time of hire to obtain a current license – California regulation provided.) Basic Life Support (BLS) certification required. Demonstrated clinical competency, experience, training and education required to perform the job. Proficiency with computer software applications commonly used in an automated patient care environment (e.g., EHR, documentation tools). Ability to use patient care equipment, evaluation tools, and therapeutic modalities. Experience with copy machine, multi‑line telephone, fax, computer, pager, and miscellaneous office equipment. Employees working for Banner Home Care/Hospice or Boswell Skilled Nursing Facility must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain it for the duration of employment.
