Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations for members at their homes. You will connect with individuals to review medical history, perform physical evaluations, and identify gaps in care. Role requires active licensure in the coverage area, board certification, and ability to travel locally. Per diem with an hourly base rate and incentive structure; part-time schedule with 12 hours weekly.” #J-18808-Ljbffr
