Required Credentials Associate’s Degree in a Physical Therapist Assistant program BLS certification from the American Heart Association upon hire Licensed Physical Therapist Assistant in the state of Illinois Qualifications Licensed physical therapist assistant who complies with the Code of Ethics/Standards of Practice as outlined by the professional organization and by the State of Illinois. Experience providing direct patient care, teaching, committee participation and departmental process improvement. Serves as a role model for all therapists and clinical interns in professional, clinical, communication, education and leadership activities. Completes daily notes that reflect patient response to treatment, actual performance, and assessment relative to the treatment plan and discharge needs; meets all documentation requirements. Collaborates with the PT to manage the course of PT from initial contact to discharge for functional progress, safety, and treatment outcomes. Adheres to department policy regarding documentation; all notes are entered into the electronic medical record. Provides educational materials and methods appropriate to the patient’s level of understanding and motivation. Participates in committees, departmental processes and special projects in accordance with clinical expertise and performance expectations. Develops and maintains professional relationships with members of healthcare teams (e.g., physician, nurse, case manager) and participates in internal and external marketing activities. Creates a culture of respectful communication by listening, seeking team member input, fostering open and honest conversation, and acknowledging contributions. Accepts constructive feedback and makes timely changes as evidenced by coaching forms and team feedback. Proactively completes non‑patient care responsibilities within established timelines (cover notes, PTO/schedule change requests, bonus pay forms, and follow‑up phone calls). Contributes to educational efforts of the department/clinic, such as in‑services, journal clubs, new hire orientation, and professional improvement. Actively participates in student clinical or observation programs by serving as a clinical instructor, mentor, or observation host. Demonstrates willingness to flex schedule or assignment to meet patient care needs, staff meetings and department meetings outside normal work hours.
