Service Coordinator

We are offering a $500 sign on bonus!! Family Service Association is seeking a part time (21 hours per week) Site Coordinator for our Countryside Adult Day Health Program in Raynham, MA. The Site Coordinator assists the Program Coordinator with management of day-to-day operations of the program, provides case management services, and monitors mental health status of program participants. The hourly rate is $21 and the hours are Monday through Friday, 10:00 a.m. - 2:00 p.m. Duties and Responsibilities: Provide case management services Develop and lead groups for the participant. Assist the program coordinator in referral, admission and discharge process. Write progress notes quarterly. Inform participants and their families/caregivers of available community services, and make referrals as indications warrant. Attend monthly supervisor meetings with the Program Coordinator (Bi-weekly with LICSW if not licensed). Assist, as necessary, in the delivery of other required program services. Participate in the bi-annual or quarterly review of each participant’s care plan as well as assisting with the development of care plans. Assist Program Coordinator with the implementation of policies and procedures. Assist Program Coordinator with record reviews, documentation, compliance, and implementation of policy and procedure. Ability to provide services outlined in this job description at any of the three Adult Day Health sites as assigned by the Program Coordinator. Education and Experience Requirements: Bachelors Degree in Health or Human Services or a related field required. At least One (1) year of recent social work experience working with disabled/elderly adults in a professional capacity is required. Significant work experience with the elderly and/or physically/mentally challenged adults in a community setting preferred. **Must be able to successfully complete a MA State CORI check, CPS check and company paid, and pre-employment physical.** Family Service Association is a comprehensive private, non-profit social service agency dedicated to the development and implementation of services designed to provide strength and support to individuals and families throughout southeastern Massachusetts.
