Patient Rights Facilitator

JOB REQUIREMENTS: As the Client / Patient Rights Facilitator, you will provide for the protection of client rights. In this position, you are responsible for the following: Function as an advocate/investigator to follow up on questions and concerns of individuals with intellectual/developmental disabilities (ID/DD). Ensure all recommendations and corrective actions are followed regarding resident rights and safety. Train and consult staff regarding rights, conduct thorough investigations, and evaluate trends/patterns for systematic changes. Serve as an advocate for individuals with ID/DD regarding ethical questions both at SWC and external facilities. Actively participate in quality improvement projects, assignments, and committee work to ensure resident rights and safety meet regulatory standards. Utilize technology to collect data, monitor and analyze incident trends/patterns for administrative review and corrective preventive actions. ***** OTHER EXPERIENCE AND QUALIFICATIONS: Minimally qualified applicants will have all of the following: - Experience working directly With People With Intellectual Or Other Developmental Disabilities. - Training or experience advocating for patient rights. - Experience using Microsoft Office Suite (e.g. Outlook, Word, Excel) for data entry, communication, and report production. - Experience collecting and analyzing data to recognize trends and make recommendations. Well-qualified applicants will also have one or more of the following: - Experience performing investigations and submitting comprehensive summary reports related to patient or client rights, or mistreatment. - Experience providing consultation, technical assistance, or training regarding patient or client rights, or grievance procedures. - Experience developing and implementing quality improvement. - Meet the minimum definition of Qualified Intellectual Disability Professional (QIDP) as defined as: * A professional who has at least one year of Experience Working Directly With Persons With Intellectual Disabilities or other developmental disabilities AND Is one of the following: (1) a doctor of medicine or osteopathy; (2) a registered nurse; or (3) an individual who holds at least a bachelor\'s degree in a professional category specified in the Federal Register as meeting the QIDP qualifications as outlined in 483.430 of the Code of Federal Regulations.
