Clinical Liaison (RN)

Role Overview The Sales Clinical Liaison is a registered nurse or advanced clinical professional responsible for guiding patients through hospice referral and admission processes, acting as a patient advocate, and ensuring appropriate care levels. Key Responsibilities Proactively develop new referral sources and build community relationships to grow the organization. Engage patients and families with compassionate education about hospice services and enrollment guidance. Coordinate with healthcare providers, maintain communication, and manage follow-ups and documentation. Educate stakeholders, analyze referral trends, and promote the organization as a community leader. Qualifications & Skills Active RN license with an associate’s or bachelor’s degree in nursing. Strong communication, relationship-building, and customer service skills. Knowledge of healthcare systems, medical terminology, and referral processes; hospice experience preferred. Organizational skills with the ability to manage multiple priorities and use electronic health records effectively. Ability to work collaboratively, demonstrate initiative, and adapt to changing needs. Preferred: clinical nursing experience in case management and hospice care knowledge.
