Work Environment

This position will be held onsite at our Goshen facility, an inviting, supportive environment where rehabilitation occurs alongside the comprehensive healthcare services we offer. You will work collaboratively within a multidisciplinary team that includes nurses, occupational and speech therapists, and physicians, all striving toward common goals to improve patient outcomes and provide an enriching experience for our residents.

Key Responsibilities

• Conduct thorough evaluations and assessments of residents' physical conditions and functional limitations to identify their specific rehabilitation needs.
• Develop and implement individualized treatment plans that integrate evidence-based practice to promote recovery, improve mobility, and enhance physical function.
• Administer various therapeutic exercises, manual therapy techniques, and modalities designed to alleviate pain and address physical impairments.
• Monitor and document patients' progress regularly, making necessary adjustments to treatment plans based on their evolving needs and recovery goals.
• Collaborate with an interdisciplinary team, participating in team meetings to discuss patient progress and refine holistic care approaches.
• Provide education to patients and their families regarding home exercise programs and strategies to support ongoing progress and recovery.
• Ensure adherence to facility protocols, state regulations, and industry best practices to maintain the highest standards of patient care.

Patient Population / Client Focus

As a Physical Therapist at Work It PT, you will primarily work with older adults facing a range of physical rehabilitation needs including post-operative recovery, stroke rehabilitation, and age-related mobility limitations. This role requires a compassionate, patient-centered approach to care, as many residents may be navigating challenging health issues and require emotional support in addition to physical rehabilitation.

Qualifications

• Education: Completion of a Doctor of Physical Therapy (DPT) program or a Bachelor's/Master's in Physical Therapy from an accredited institution.
• Licensure: Possession of an active and unrestricted Physical Therapy license in the state of New York is required.
• Skills: Strong clinical skills paired with excellent communication and interpersonal abilities to effectively collaborate with patients, families, and team members.
• Approach: A commitment to providing patient-centered care that demonstrates compassion and empathy in all interactions.

Preferred Experience

• A minimum of 2 years of experience as a Physical Therapist, particularly within a skilled nursing facility or rehabilitation center, is strongly preferred.

• Familiarity with interdisciplinary collaboration in a rehabilitation setting to optimize patient outcomes.
