QIDP Case Manager — Home & Community-Based Support
Envision Unlimited
Envision Unlimited in Illinois is seeking a full-time caseworker to provide direct services to adults with developmental disabilities. You will coordinate service authorizations, maintain case records, and advocate for members in the community. Requirements include a bachelor's degree in social/human services, at least 1 year of relevant DD experience, QIDP eligibility, and a valid driver's license with ability to travel up to 70% in the Chicagoland area.
