Min USD $36,000.00/Yr. Max USD $39,784.00/Yr. Position Overview S:US is seeking a Medication Coordinator for our mens shelter. This position is located at Ralph Avenue, Brooklyn, NY 11233 The Medication Coordinator administers medications and documents the administration as required. ESSENTIAL DUTIES & RESPONSIBILITIES: Administer medications and document administration as required. Monitoring of MARs and medication storage. Work with residents and Case Managers in filling prescriptions and ensuring refills are ordered as necessary. Ensure physician orders are current, properly documented and communicated to staff Monitor client medical equipment for safety and proper functioning. Attend and participate in regular staff training and scheduling meetings. Maintain confidentiality and privacy regarding all clients’ information Complete daily documentation of all agency required material. Complete incident reports in accordance with policy and procedures. Understand and implement the agency’s philosophy, policies and mission. Understand and follow the agency chain of communication. Ensure the safety and well-being of the clients on a daily basis. Work cooperatively with other team members to carry out the program goals and objectives for each client. Report any actual or suspected abuse/neglect in accordance with policy and procedure. Assist the psychiatrist and program nurses Prepare clients for self-medication status and monitor as needed, health assessment referrals. Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes. Maintain CPR/First Aid Certification. Assist in building evacuation during fire drills and building emergencies. Refer residents to emergency care if needed, work with emergency responders/emergency room staff to coordinate emergency treatment, coordinate treatment and discharge planning with hospital staff. Additional duties as assigned Qualifications REQUIRED EDUCATION AND EXPERIENCE High School Diploma or GED At least one year of experience in direct clinical work with mentally ill populations. Preferred experience would include previous work with homeless population; work in residential settings and/or community health care, patient education/treatment adherence counseling and group work Ability to work independently and as part of a team, to assist adult males of diverse ethnicities, backgrounds, ages, and sexual preferences.
