Mobile Geriatric PCP for Senior Living Communities

Twin Cities Physicians is seeking a Physician (MD/DO) to provide on-site mobile primary care and geriatric rounding across Minnesota senior living communities. You will manage chronic diseases, coordinate post-acute care, and prevent avoidable hospitalizations while delivering compassionate care in residents' homes and communities.
The ideal candidate is licensed in Minnesota, has geriatrics or primary care experience, and is able to travel with a valid driver's license to multiple sites.
