Job Summary BAYADA Home Health Care is seeking a Registered Nurse (RN) Home Health Admission for a nursing job in Burlington, Massachusetts. This is a full‑time, ongoing position focused on completing OASIS Start of Care (SOC) visits for adult and geriatric patients across northeastern Massachusetts. Responsibilities Perform OASIS Start of Care (SOC) visits for adult and geriatric patients in the home setting throughout northeastern Massachusetts. Conduct comprehensive head‑to‑toe assessments and identify subtle changes in patient condition. Triage and manage urgent clinical situations independently. Perform medication reconciliation, teaching, and safety monitoring. Complete wound assessments, provide wound care, and document appropriately. Utilize acute‑ and post‑acute care experience (e.g., CHF, COPD, wound care, IV therapy, post‑surgical care) to deliver quality care. Maintain efficient and accurate point‑of‑care documentation using required technology. Manage a dynamic visit schedule, prioritize urgent visits, and adjust to changing patient needs. Qualifications Current nursing license in good standing in Massachusetts. Graduation from an accredited and approved nursing program, verified by transcript or diploma. At least one year of recent hands‑on medical experience with an adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med‑surg, sub‑acute care, or rehab units. Ability to work independently while knowing when to seek escalation. Comfortable making real‑time clinical decisions without immediate on‑site support. Strong problem‑solving skills in unpredictable home environments. Experience handling dynamic visit schedules and prioritizing urgent care. Attention to detail while meeting productivity expectations. Cultural sensitivity and respect for patient autonomy. Adaptability to changing patient needs and schedules. Proficiency with Medicare coverage requirements and OASIS documentation. Strong point‑of‑care documentation skills with relevant technology.
