Job Title & Location Registered Nurse (RN) – Home Health Admission (SOC) – Westford, Massachusetts (and surrounding towns). Overview Full‑time, ongoing staff position in home health admissions. Responsibilities Complete OASIS Starts of Care (SOC) for adult and geriatric patients. Accurately document observations, interventions, and evaluations related to client care management using a touchscreen tablet. Qualifications Current license as a Registered Nurse in Massachusetts. Graduation from an accredited nursing program. Medicare‑certified home health experience required. Compensation Base pay $58.00 – $65.00 per point, depending on qualifications.
