Job Summary As a Street Outreach Specialist you will find, connect with, and enroll eligible individuals into our Enhanced Care Management (ECM) and Community Supports (CS) program. This role focuses on building trust and guiding people toward a system of support, rather than a hard sell. Duties / Responsibilities Drive enrollment & conversion: manage the full engagement cycle from initial contact to successful onboarding. Generate leads & build your pipeline through proactive canvassing of communities. Qualify prospects with precision using DHCS eligibility criteria to ensure efficient use of time. Build a high‑value referral network with partner agencies, community leaders, and organizations. Maintain your CRM and report on metrics to measure success and refine outreach strategy. Perform other duties as assigned. Education and Experience High School Diploma or GED required. At least one year of direct outreach, sales, or community engagement experience. Experience working with unhoused or high‑need populations is highly desirable. Required Skills / Abilities Excellent verbal & written English communication skills with empathy, persuasion, and clarity. Bilingual skills, particularly Spanish, are desirable. Knowledge of community and county resources. Proficiency with modern tech tools: laptops, smartphones, Microsoft Office, email. Street‑smart and empathetic interpersonal skills. Sales‑acumen and drive to meet, exceed enrollment targets. Valid California driver’s license, personal auto insurance, and an MVR sufficient for agency liability. Compensation & Incentives Performance‑based bonus structure tied to enrollment targets. Commission on each successful enrollment with an uncapped plan. Tiered achievement bonuses for member engagement and retention milestones. Competitive base salary for financial stability. Physical Requirements Mostly community‑based work (approximately 75%) with 25% office presence; safety precautions may be required. Ability to lift a minimum of 15 lbs. Prolonged periods of sitting at a desk on the computer. Pay range: $25 USD – $28 USD per hour.
