Qualifications (What You Bring)

• Graduate from an accredited nursing program; a Bachelor’s degree is preferred.
• Active RN license in the state of Colorado.
• At least two (2) years of nursing experience, including long-term care, dementia care, or acute care; home care experience is a plus.
• Current CPR certification.

Key Responsibilities

The Registered Nurse (RN) is responsible for case management, coordination, supervision, and the delivery of professional and supportive services to clients, utilizing nursing theory and processes in line with the Nurse Practice Act.

• Evaluates client referral information and addresses requests and inquiries as needed.
• Conducts client assessments according to agency standards to ensure effective home care services.
• Independently directs, plans, and initiates appropriate actions in home care scenarios.
• Performs duties in compliance with agency policies, procedures, and professional standards.
• Undertakes additional responsibilities as assigned by the Director of Clinical Services.

Our organization is a Medicare/Medicaid-certified home health care agency located in Montrose, CO, featuring a skilled team of nurses, rehabilitation therapists, and home health aides dedicated to meeting individual health care needs with compassion and understanding. We specialize in caring for older adults, providing in-home health care services across Delta, Montrose, and Ouray counties.

Experience

• Required: 2 years of nursing experience, including a mix of long-term care, dementia care, or acute care.
• Preferred: Experience in home care.

Education

• Preferred: Bachelor’s degree in Nursing or a related field.

Licenses & Certifications

• Required: CPR/First Aid certification
• Required: Registered Nurse license
