Qualifications (What You Bring)

• Graduate from an accredited nursing program; a Bachelor’s degree is preferred.
• Active RN license in the state of Colorado.
• At least two (2) years of nursing experience, including long-term care, dementia care, or acute care; home care experience is a plus.
• Current CPR certification.

Responsibilities (What You’ll Be Doing)

As a Registered Nurse (RN) , you will manage cases, coordinate care, supervise, and implement professional and supportive services for clients, utilizing nursing theory and processes in line with the Nurse Practice Act.

• Evaluate client referral information and respond to inquiries as needed.
• Conduct client assessments according to agency standards to ensure effective home care services.
• Independently direct, plan, and initiate appropriate actions in home care situations.
• Carry out job responsibilities in accordance with agency policies, procedures, and professional standards.
• Perform additional duties as assigned by the Director of Clinical Services.

Our organization is a Medicare/Medicaid-certified home health care agency located in Montrose, CO, featuring a skilled team of nurses, rehabilitation therapists, and home health aides dedicated to meeting individual health care needs with compassion and empathy. We specialize in caring for older adults, providing in-home health care services across Delta, Montrose, and Ouray counties.

Experience Required

• 2 years of nursing experience, including a mix of long-term care, dementia care, or acute care.

Preferred Experience

• Experience in home care.

Preferred Education

• Bachelor’s degree in Nursing or a related field.

Required Licenses & Certifications

• CPR/First Aid certification.
• Registered Nurse license.
