Responsibilities: Provide direct patient care in home health settings under the supervision of an RN or physician Administer medications, treatments, and wound care as prescribed Monitor and document patient health status, vital signs, and progress Develop and follow individualized patient care plans Communicate effectively with patients, families, and the broader care team Educate patients and caregivers on health management and best practices Ensure compliance with state regulations and company policies in home health care Requirements: Active LPN license in the state of Michigan Previous home health, long-term care, or clinical nursing experience preferred Strong knowledge of medication administration and wound care protocols Excellent communication, documentation, and organizational skills Reliable transportation and valid driver's license Compassionate, patient-focused demeanor with a commitment to quality care CPR/BLS certification required or ability to obtain upon hire About Us: ComForCare Home Health Care - West Bloomfield is a trusted provider of in-home health care services dedicated to helping clients live their best lives possible in the West Bloomfield Township area.
