Sunrise Hospital & Medical Center in Las Vegas, NV is seeking a skilled RN Case Manager PRN to coordinate patient care, manage length of stay, and develop safe discharge plans in collaboration with the multidisciplinary team. You will perform admission reviews for medical necessity, maintain admission monitoring, coordinate with hospital staff and community resources, and ensure timely referrals to Home Health and related services to optimize patient outcomes. #J-18808-Ljbffr
