Registered Nurse Case Manager (RN) – Acute Care

Location: Las Vegas, NV
Salary: $40.72 – $63.12 per hour + Benefits
Full-Time | Permanent
Sign-On Bonus: Up to $15,000 Eligible

Position Summary:

The Nurse Case Manager (RN) coordinates and manages patient care throughout the healthcare continuum using a collaborative, multidisciplinary approach. This role serves as a clinical resource for patients, families, physicians, and healthcare staff to ensure quality outcomes, effective discharge planning, and continuity of care.

Key Responsibilities:

• Coordinate patient care and discharge planning
• Conduct patient assessments and care evaluations
• Collaborate with physicians and multidisciplinary teams
• Monitor utilization review and care management processes
• Ensure compliance with hospital and regulatory standards
• Educate patients and families regarding treatment plans and resources
• Maintain accurate clinical documentation

Qualifications

• Graduate from an accredited School of Nursing
• Active Nevada RN License required
• BLS Certification through AHA required
• Minimum 3 years of acute care nursing experience
• Minimum 1 year of Case Management, Discharge Planning, or Utilization Review experience
• Strong communication, organizational, and clinical assessment skills

Preferred Qualifications

• Pediatrics Case Management experience
• Emergency Department experience
• Utilization Review experience
• Charge Nurse experience
• CCM or ACM certification preferred
