Nurse Case Manager (RN)

Location: Las Vegas, NV

Job Type: Full-Time

Salary: $86,000 - $133,000 + Up to $15,000 Sign-On Bonus

Position Overview:

We are seeking an experienced Registered Nurse Case Manager to coordinate and manage patient care across the healthcare continuum in a fast-paced acute care hospital setting. This role collaborates with physicians, nurses, patients, and multidisciplinary teams to ensure high-quality, cost-effective care and safe discharge planning.

Key Responsibilities:

• Assess, plan, coordinate, and evaluate patient care needs.
• Develop and manage individualized discharge and transition-of-care plans.
• Collaborate with multidisciplinary teams to optimize patient outcomes.
• Perform utilization review and ensure appropriate level of care.
• Educate patients and families on care plans and available resources.
• Maintain accurate documentation in the electronic medical record.

Qualifications:

• Active Nevada Registered Nurse (RN) license.
• BLS certification (AHA required).
• Minimum 3 years of acute care nursing experience.
• At least 1 year of Case Management, Discharge Planning, or Utilization Review experience.
• Knowledge of MCG (Milliman) guidelines preferred.
• CCM or ACM certification is a plus.
