Leading a team of Transitions of Care Associates, the remote Supervisor will ensure high-quality patient support during post-hospital transitions, focusing on coaching, operational excellence, and collaboration with clinical partners while dedicating 30% of the time to direct patient care. Key responsibilities Lead, coach, and mentor a team to meet performance, quality, and patient experience goals Conduct regular meetings and coaching sessions to foster skill development and team engagement Monitor key performance indicators and use data to identify trends and improvement opportunities Required qualifications Active and clear Medical Assistant Certification 2+ years of direct lead or supervisory experience in a virtual setting Experience with transitional care or care coordination Strong organizational skills and attention to detail Bilingual in English and Spanish preferred
