- Licensed Practical Nurse - WheelerWheeler - Tyngsboro, MA 01879## OverviewSalary Range$35.75 - $35.75 Hourly## Description**This is a bargaining Unit position****Summary**Monitors the health, safety and well-being of participants in the respite home. Incumbent is responsible for overseeing and managing the medical and health care of program participants, including providing direct nursing care. Incumbent reports issues to appropriate people and handles emergencies according to Incompass and other applicable agencies guidelines. Incumbent must comply with applicable regulatory documentation and standards in the performance of nursing duties. This is a bargaining unit position.**Essential Job Functions*** Monitors the health of program participants, including preventative care, and provides professional technical assistance regarding nursing care and medical treatment as needed.* Responsible for management and documentation as it relates to medical information, referrals, reports, medication administration and data collection.* Responsible for the completion of medical record audits and review of medical tracking systems.* Provides oversight and assistance with the self-medication assessment process.* Provides oversight of MAP and for the administration of medical treatments and staff training as needed including annual mandated staff training. Administration is done in accordance with Nurse Practioner’s Act as well as DDS and MAP regulations.* Helps when needed regarding medication supplies and medical supplies as needed.* Orients and trains’ staff in understanding participants’ medical needs and in following prescribed programs and plans. Communicates with and trains staff to complete procedures as appropriate.* Provides leadership and support to team during medical crisis.* Ensure professional communication with families, DSS, clinicians and others for assigned participants.* Provides guidance to staff regarding medical emergencies and informs supervisor of all emergency treatments or 911 calls made.* Communicate with medical providers regarding major issues.* Completes nursing notes and documentation for encounters with individuals.* Attend meetings as assigned* Serves as a mandated reporter as required by the Disabled Persons Protection Commission.* Processes all human rights issues, incident and accident reports in a timely manner according to agency policy/procedure. Ensures the human rights of program participants are safeguarded in all interventions.* Responsible for knowing and following agency, departmental, program and site policies and procedures as well as the bargaining unit personnel policies handbook and the collective bargaining agreement.* Fosters and maintains positive relationships with families, participants and staff and informs supervisors of major and/or unresolved issues.* Acts in a manner consistent with organizational values, program mission, and service delivery goals. Serves as a resource and role model for staff.* Completes other duties as assigned by supervisor as they relate to the duties in this job description.*This job description is not all inclusive and does not state or imply that these are the only duties to be performed by the incumbent.
