Lead Registered Nurse Assessment Coordinator (MDS) - Philadelphia, PA
Overview
Caring Heart Rehabilitation and Nursing Center, a skilled rehabilitation and nursing facility located in Philadelphia, Pennsylvania, is seeking an experienced and dedicated Lead Registered Nurse Assessment Coordinator (MDS) to join our team. As a leader in short-term and long-term skilled rehabilitation, we provide exceptional care to our patients and residents. Job Summary
The Lead Registered Nurse Assessment Coordinator (MDS) is responsible for the timely and accurate completion of the Minimum Data Set (MDS) assessments and coordination of the Resident Assessment Instrument (RAI) process in accordance with federal, state, and facility guidelines.
Responsibilities
Schedule and lead interdisciplinary team (IDT) care plan meetings
Implement and interpret MDS (Minimum Data Set) assessments
Review medical records, nursing documentation, and therapy notes to validate MDS accuracy
Accurately code MDS Items to reflect resident acuity, ensuring appropriate PDPM classification
Track and manage Case Mix Index (CMI) and Quality Measure data to support facility reimbursement and survey readiness

Current PA RN license
Minimum 2 years of experience in MDS or RNAC role
