Kidney Care Options Educator, Care Coordinator
U.S. Renal Care
U.S. Renal Care seeks a Kidney Care Options Educator / Care Coordinator to guide patients with chronic kidney disease through treatment choices and care planning. In this role, you will educate patients and families on dialysis, transplant, and home therapies; coordinate referrals and appointments; and collaborate with nurses, social workers, and physicians to support seamless care transitions. You will track patient progress, maintain accurate documentation, and provide compassionate support that reflects our patient-first culture. This position offers the opportunity to grow within a mission-driven, interdisciplinary kidney care team.
