Key Responsibilities\n

• Conduct thorough psychosocial assessments through interviews, observational techniques, and review of medical records to evaluate the emotional, social, and environmental factors that impact students' recovery.

• Provide individual and family counseling sessions focused on addressing adjustment issues, emotional distress, and coping strategies related to health challenges.

• Work closely with the rehabilitation team to formulate and implement individualized treatment plans that cater to the psychosocial needs of each student.

• Guide students and their families in accessing community resources and services, such as support groups and vocational rehabilitation programs.

• Educate families about community resources and post-rehabilitation options to ensure a smooth transition back to school and home environments.

• Participate in discharge planning, facilitating effective transitions for students returning home or moving to other care settings.

Our primary focus is on school-aged children and adolescents who are dealing with physical and mental health challenges that affect their educational experience. Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.
