Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through interviews, observations, and thorough review of medical records to evaluate the emotional, social, and environmental factors that may impact patients' recovery.

• Provide empathetic counseling and ongoing support to patients and their families, helping them navigate adjustment issues related to their rehabilitation journey.

• Collaboratively work with a multidisciplinary rehabilitation team to formulate individualized treatment plans that incorporate psychosocial needs and goals.

• Assist patients in identifying and accessing community resources such as local support groups, vocational rehabilitation services, and housing assistance programs to support their transition back to everyday life.

• Engage in discharge planning, facilitating smooth transitions for patients moving to home or alternative care settings while ensuring continuity of care is maintained.

Patient Population / Client Focus\n

Our clients include individuals recovering from a variety of conditions such as strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Qualifications\n

Compensation and Benefits\n
