Key Responsibilities\n

• Conduct thorough clinical assessments to better understand clients’ mental health needs and develop individualized treatment plans.

• Facilitate therapy sessions for patients, focusing on end-of-life transitions, grief counseling, and emotional support.

• Utilize our user-friendly electronic health record (EHR) system for client management, scheduling, and billing purposes.

• Collaborate with other healthcare providers including nurses and doctors within the hospice setting to ensure comprehensive patient care.

• Provide referrals to community resources and support services as necessary to address clients’ needs.

• Engage in ongoing professional development and attend scheduled peer support groups with other licensed providers.

Your practice will primarily focus on individuals receiving hospice care and their families, addressing the psychological and emotional challenges that arise during end-of-life stages. Qualifications\n

• A Master's or Doctorate level degree in Social Work from an accredited institution.
