Physical Therapist

Role: Licensed Physical Therapist providing in-home, senior living, and virtual care to older adults, primarily through one-on-one sessions. Responsibilities: Conduct evaluations, develop personalized treatment plans, and deliver evidence-based therapy to improve mobility, independence, and quality of life. Document patient progress accurately within EMR systems and collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists. Educate patients, caregivers, and staff to promote safety and aging in place. Incorporate telehealth and virtual care for increased flexibility and patient access. Qualifications & Skills: Valid Physical Therapy license or eligibility in practice state(s). Degree from an accredited PT program and proficiency with EMR systems. Current CPR certification. Physical Requirements: Ability to exert moderate effort, assist with patient transfers, perform CPR, and drive to various care settings.
