Work Environment\n

This position offers a flexible remote work environment, which allows for a better work-life balance. As a school-based social worker, you will primarily support seniors attending various educational programs or activities. Your work will involve occasional visits to school facilities, ensuring that support services are accessible for both seniors and their families. Full-time work hours are Monday through Friday from 8:00 AM to 4:30 PM, with the potential for occasional evening or weekend hours based on client needs.

Key Responsibilities\n

• Conduct in-depth psychosocial assessments of seniors to evaluate emotional, mental, and social needs.

• Develop and implement personalized care plans in collaboration with clients and their families, focused on achieving individual goals.

• Provide counseling services to seniors and their families, guiding them through life transitions and various challenges.

• Advocate for clients’ rights, ensuring they have access to necessary community resources, services, and support.

• Work in concert with healthcare professionals, including nurses and caregivers, to coordinate comprehensive care.

• Perform regular check-ins with clients to assess their well-being and living conditions, promptly addressing any concerns that arise.

• Facilitate effective communication among family members involved in the senior's care, addressing conflicts when necessary.

• Educate clients and their families about community resources, support networks, and programs to promote social engagement.

• Maintain thorough documentation of all client assessments, interactions, and progress notes while adhering to ethical and legal standards.

• Stay current on best practices, policies, and regulations related to social work and elder care.
