Work Environment\n

This position is fully remote, with a focus on school settings where you will have the flexibility to connect with clients and families via telehealth platforms, as well as on-site visits as necessary. You will engage with a team of dynamic professionals, including educators, health care providers, and other social workers, all working collaboratively to provide comprehensive support to seniors in Tallahassee.

Key Responsibilities\n

• Conduct comprehensive psychosocial evaluations of seniors and work with interdisciplinary teams to create individualized care plans tailored to their unique needs and goals.

• Deliver one-on-one counseling sessions to seniors, utilizing evidence-based therapeutic techniques to address emotional, social, and mental health challenges.

• Facilitate resource navigation by providing clients and their families with information about community resources, support services, and programs available to seniors.
\n
• Champion clients’ rights and ensure they have access to necessary services, advocating on their behalf when challenges arise.

• Monitor and assess clients’ progress through regular check-ins, home visits, and documentation, ensuring all assessments are systematically recorded in electronic health record (EHR) systems.

• Develop and deliver educational workshops on relevant topics related to aging, mental health, and social engagement for clients and family members.

• Coordinate with caregivers and health care professionals to create supportive environments that enhance the well-being of clients.

• Stay abreast of the latest social work best practices, legislative changes, and geriatric care philosophies to ensure compliance and ethical standards are upheld.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is essential.

• A current and valid LMSW license issued by the state of Florida is required.

Compensation and Benefits\n
