Clinic Care Coordinator-RN

Good Samaritan

Good Samaritan Health Center in Atlanta seeks a Clinic Care Coordinator-RN to support whole-person, affordable care for underserved patients. The RN will coordinate care across primary, dental, behavioral health, and wellness services; perform triage and assessments; develop care plans; and provide patient education. This on-site role collaborates closely with an interdisciplinary, faith-inspired team, advancing health equity and quality outcomes while documenting thoroughly in the EHR and supporting continuous improvement and mission-centered service.
