Provides clinical social work services to assigned patient care areas, supporting patients, families, and care teams in addressing psychosocial needs and barriers to care. Develops and implements plans to help resolve or reduce issues affecting patient care, in collaboration with patients, families, physicians, staff, and community resources. Provides patient education, crisis intervention, short-term counseling, discharge planning, and referrals to community and government agencies. Documents assessments, interventions, care plans, and follow-up in the medical record in a timely and accurate manner. Responds to urgent and unplanned situations by assessing needs, taking appropriate action, and notifying management as needed. Master’s degree in Social Work required Current Maryland social work licensure or license eligibility at the LGSW, LCSW, or LCSW-C level Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
