Job Responsibilities:

• Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards.
• Collaborate with multidisciplinary team members to optimize patient care and outcomes.
• Educate patients and their families on health maintenance and disease prevention.
• Ensure accurate documentation and compliance with all regulatory guidelines.
• Case Management for other CNAs, CHHAs.

Requirements:

• Active RN license in the state of practice.
• Previous experience in a home care or acute care setting.
• This is a full-time, in-person role, requiring presence in the office 3 days per week.
• Responsible for administrative duties, field RN cases, oversight, assessments, and coordination of care.
• Balance office responsibilities with field support while maintaining leadership accountability.
• Exceptional interpersonal and communication skills.
• Committed to delivering high-quality, patient-centered care.
