RN Case Manager (Home Health) – RN Case Manager

Location: Pensacola, FL (On-site + field visits in Escambia, Santa Rosa, Okaloosa, Walton)
Schedule: Monday–Friday 8:00 AM–5:00 PM + 1 weekend per month + 1 on-call night per week
Mileage: $0.43 per mile

Summary

Hiring an experienced RN Case Manager to complete in-home assessments for high-risk members and drive safe, cost-effective care transitions across the Pensacola area. This RN Case Manager role is ideal for a home health nurse who is confident with OASIS documentation and Homecare Homebase (HCHB)/PointCare , collaborates well with physicians and multidisciplinary teams, and can manage a territory-based caseload with strong clinical judgment and tight documentation standards.

Responsibilities

• Perform comprehensive assessments for high-risk members, identifying clinical, functional, and psychosocial needs
• Collaborate with primary care providers to implement individualized care plans and targeted interventions
• Reassess level of care for appropriateness and cost efficiency; identify barriers to compliance (transportation, housing, support needs)
• Coordinate transitions of care with physicians, social workers, discharge planners, and other stakeholders
• Engage specialty resources to resolve complex cases and prevent avoidable adverse outcomes
• Document clinical, functional, and fiscal outcomes accurately and timely in the system
• Provide patient advocacy while protecting privacy, confidentiality, and regulatory compliance
• Support field-based care management across Okaloosa and Walton counties (and surrounding coverage area as needed)

Qualifications

• Current, unrestricted Registered Nurse (RN) license; Florida license required (compact accepted)
• Nursing degree (ADN or BSN)
• Strong home health documentation skills: OASIS (required)
• System experience: Homecare Homebase (HCHB)/PointCare (required)
• Preferred clinical skills: wound care, PICC/IV care , lab collection/phlebotomy, drain care, ostomy care , post-surgical care
• Ability to analyze complex medical information, prioritize effectively, and make sound clinical decisions
• Strong written/verbal communication and professional customer-facing approach in the home health environment
• Ability to travel throughout assigned counties and manage a field schedule reliably
