Description

• Conduct evaluations of patients in either inpatient or outpatient environments and create personalized treatment plans
• Apply physical therapy interventions aimed at improving mobility, strength, balance, and overall function
• Collaborate with nursing staff, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
• Inform patients and their families about therapeutic goals, discharge planning, and home exercise programs
• Record evaluations, treatment sessions, and progress notes following facility and regulatory guidelines
• Engage in care team rounds and interdisciplinary meetings to foster coordinated care

Education

• Doctor of Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification preferred
