Hospice RN Case Manager — Compassionate Care & Planning

Optum’s Casa de la Luz East Valley hospice team seeks a compassionate RN Case Manager in the Phoenix area. You’ll manage Plan of Care, evaluate care quality, and coordinate treatment with an interprofessional team to support patients and families facing end-of-life decisions. Current unrestricted RN licensure and a valid driver's license are required; CPR certification or ability to obtain within 90 days is preferred.
