Hospice RN Case Manager

Role Overview The Registered Nurse Case Manager is responsible for delivering comprehensive hospice care, developing and implementing personalized Plans of Care in collaboration with patients, families, and healthcare teams. Key Responsibilities Conduct initial evaluations within 48 hours of referral to determine hospice eligibility. Assess and re-assess patients' physical, psychosocial, and environmental needs at least every 15 days. Develop and coordinate individualized care plans, documenting goals, interventions, and responses. Collaborate daily with patients, families, physicians, and interdisciplinary team members. Educate and supervise patients and families in self-care techniques when appropriate. Maintain accurate clinical documentation of patient conditions. Adapt to diverse home environments and patient backgrounds. Qualifications & Skills Current, unrestricted RN licensure. Valid driver’s license, vehicle insurance, and reliable transportation or access to public transit. Preferred: 1+ years of clinical experience and CPR certification (or ability to obtain within 90 days).
