Description: The Registered Nurse implements professional nursing practice centered on the patient as a whole person. The role focuses on creating a caring environment centered on healing and wellness. Nursing practices are based on evidence, research, and established policies and procedures. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships with nursing staff and other disciplines. This position involves coordinating, ensuring, and providing direct patient care activities on the unit. Responsibilities include conducting nursing rounds, assessments, and treatments. Facilitating admission, transfer, and discharge activities is also essential. Providing patient and family-centered education is a critical aspect of the role. Care coordination and individualized treatment planning are conducted as applicable. The RN performs and documents comprehensive assessments in accordance with the scope of practice. Consideration is given to the patient’s culture, age, and developmental level. Identifying patient risk factors and obtaining health information from family or others is required. Collecting objective and subjective data is essential for holistic care. The RN develops a problem list, prioritizing patient needs based on inter-professional team input. Actions and interventions are designed based on the patient’s culture and age. Desired outcomes are determined in coordination with the patient and family in accordance with the medical plan of care. Patient care therapies and procedures are fully implemented with clinical autonomy. The RN explains procedures and treatments to patients and families. Initiating action to alleviate patient stress and discomfort is part of the role. Recognizing, responding to, and communicating changes in patient conditions is essential. Ongoing assessments and responses to procedures and therapies are conducted according to policy and established standards. The RN delegates tasks appropriately and oversees team members in patient care activities. Monitoring the accuracy and completeness of delegated patient care is vital. Identifying and addressing educational needs of patients is part of individualized care planning. Facilitating the patient discharge process is conducted in a timely manner. Providing input into patient/nurse assignments considering available staffing is expected. Demonstrating active participation in unit-based activities supports organizational values. Promoting a positive group dynamic and healing environment through actions is encouraged. Assuring nursing excellence in accordance with accepted standards and evidence-based practice is mandatory. Active participation in quality improvement activities and core measures is part of the job. Using a holistic perspective regarding patient care is essential.
