Home Care RN: Community Health & Care Coordination
Pace Ri
PACE-RI is seeking an RN for home care responsibilities including safety assessments, acute care needs, medication reconciliation, and safe transitions of care. The RN collaborates with the IDT and coordinates with the Team PCP and social worker to ensure comprehensive participant care. The role involves creating and updating care plans, attending meetings, and providing education and support to participants and caregivers, with weekend/holiday coverage as required. #J-18808-Ljbffr Pace Ri
