Successful candidate must reside in Downtown New Orleans area and be willing to commute./ppAs a Care Transition Coordinator, you play a key role in helping patients safely and seamlessly transition from hospital care to home infusion therapy. This position requires excellent relationship-building, patient education, and care coordination skills. The Care Transition Coordinator works closely with hospital discharge planners, physicians, and the InfuCare Rx team to ensure patients receive timely, appropriate, and high-quality infusion care at home. This role reports to the Area Sales Director and requires some travel to hospitals, clinics, and community partners./ppAs a Care Transition Coordinator, you will play a dual role driving business development through relationship-building with healthcare partners while supporting seamless transitions of care for patients requiring home infusion therapy.
