NOW HIRING: SOCIAL SERVICES DIRECTOR

Aventura at the Bay

Aventura at the Bay is seeking a compassionate, knowledgeable, and dedicated Social Services Director to support the emotional and psychosocial well-being of our residents.

The Social Services Director will serve as an important advocate for residents and families while working closely with the interdisciplinary team to ensure residents receive the support, resources, and services they need throughout their stay.

• Provide social services and support to residents and their families.

• Complete psychosocial assessments and develop individualized social service plans.

• Serve as an advocate for residents' rights, needs, preferences, and concerns.

• Assist residents and families with adjustment to the facility and changes in care needs.

• Participate in care plan meetings and collaborate with the interdisciplinary team.

• Maintain accurate and timely documentation in accordance with facility and regulatory requirements.

• Assist residents and families with discharge planning and coordination of community resources.

• Provide support and guidance to residents experiencing emotional, social, or adjustment concerns.

• Address resident and family concerns and communicate issues appropriately to leadership.

• Coordinate with community agencies, healthcare providers, and other resources as needed.

• Assist with admissions, transfers, and transitions of care.

• Help promote resident satisfaction, engagement, and quality of life.

• Maintain confidentiality and comply with HIPAA and applicable regulations.

• Participate in facility initiatives, meetings, and quality improvement activities.

• Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or a related field preferred.

• Previous experience in social services, long-term care, skilled nursing, healthcare, or a related setting .

• Knowledge of resident rights, discharge planning, and community resources.

• Strong communication, interpersonal, and problem-solving skills.

• Compassionate and professional approach when working with residents and families.

• Strong organizational and documentation skills.

• Ability to work effectively with an interdisciplinary team.

• Ability to manage multiple priorities in a fast-paced environment.

• Proficiency with Microsoft Office and electronic medical record systems.

• Knowledge of applicable long-term care regulations and requirements preferred.
