Key Responsibilities

• Deliver high-quality, individualized physical therapy by managing the complete treatment process for an assigned group of patients.
• Conduct thorough patient assessments and participate in developing and implementing patient-centered care plans that emphasize goal achievement and positive outcomes.
• Provide and oversee the majority of the physical therapy services to ensure continuous support throughout the rehabilitation process.
• Perform evaluations and treatment interventions according to legal scope of practice and ethical standards within the profession.
• Assess and document therapeutic, rehabilitative, and functional status to assist in developing a comprehensive plan of care.
• Deliver purposeful therapeutic care directly in patients' homes, employing techniques aligned with the physician's care plan.
• Educate patients, families, and caregivers in the application of therapies, exercise regimens, and the use of therapeutic devices outside of therapy sessions.
• Manage case prioritization, determine daily staffing needs, and coordinate patient assignments to optimize service delivery.

Qualifications

• Current licensure as a Physical Therapist in California.
