Work Environment

This position is primarily onsite as part of our home care services, allowing you to deliver personalized therapy in the comfort of our patients' homes. With a Monday to Friday work schedule from 9 am to 5 pm, you will enjoy a routine that promotes work-life balance while still fulfilling your professional aspirations.

Key Responsibilities

• Conduct comprehensive initial assessments to evaluate each patient's condition, medical history, and functional limitations. This will involve using various assessment tools and techniques to understand their needs fully.
• Develop individualized plans of care based on assessment findings, ensuring that treatment goals are aligned with the patients' personal aspirations.
• Provide education and guidance to patients to enhance their understanding of their conditions and treatment plans, fostering agreement and commitment to the therapeutic course.
• Implement treatment strategies that incorporate manual therapy techniques, progressive exercise prescriptions, and interventions tailored to facilitating optimal recovery.
• Monitor patient progress closely, adjusting treatment plans as necessary to meet evolving needs and promoting accountability for patient-led recovery.
• Maintain thorough documentation of patient care, treatment rendered, and progress notes to ensure compliance with regulatory standards and provide clear communication with other health care professionals involved in the patient's care.

Patient Population / Client Focus

As a home care Physical Therapist, you will work with a diverse patient population focused primarily on orthopedic rehabilitation. Your expertise will help patients recover from surgeries, injuries, and chronic conditions, ultimately enhancing their quality of life. By working closely with each individual, you'll not only address their physical limitations but also instill confidence and motivation to engage in activities that matter most to them.

Qualifications

• Doctoral Degree in Physical Therapy (DPT) from an accredited program.
• Current licensure in the State of California as a Physical Therapist.
• Unblemished record with no instances of board actions or disciplinary measures taken against your license.

Preferred Experience

• A minimum of 3 years of experience in orthopedic physical rehabilitation.
• Expertise in manual therapy techniques that can effectively address musculoskeletal issues.
• Proficiency in developing tailored exercise regimens that support rehabilitation goals and patient engagement.
