Pride health is actively hiring a Registered Nurse to join our client's home health (Hospice) program in a growing care team serving patients .

Compensation:

• Local Pay: $55.00 to $65.00/hour (W-2)
• Travel Pay: $2,500 to $3,000 gross per week.

Location: Washington, DC, 28304

Shift schedule (days, start & end times): Friday, Saturday & Sunday (3 Days per week, 12Hours shift)

Responsibilities:

• Provide comprehensive hospice case management by coordinating and delivering patient-centered end-of-life care, completing admissions, developing and updating individualized care plans, collaborating with the interdisciplinary team, and maintaining accurate PointCare documentation while meeting productivity expectations.
• C onducting home visits, monitoring patient conditions, educating patients and families, ensuring timely interventions, and maintaining CPR certification and compliance with hospice care standards.
