Work Environment\n

This position is primarily remote, allowing for flexibility while ensuring the delivery of high-quality care to our clients in hospice settings. Our dedicated social workers operate from a hospice center, collaborating closely with an interdisciplinary team that includes nurses, caregivers, and other healthcare professionals. You will conduct virtual assessments and provide counseling, ensuring that our seniors receive timely and efficient support tailored to their individual needs. You will also have the opportunity to schedule in-person visits as necessary to assess client situations directly.

• Educate clients and families about community resources available to them, fostering social connection and engagement.

• Maintain detailed and accurate client documentation, including assessments, care plans, and progress notes in alignment with regulatory standards.

• Stay informed of current practices and policies in social work, ensuring adherence to ethical standards in all interactions and interventions.

This role focuses primarily on seniors receiving care in hospice settings. Qualifications\n

Compensation and Benefits\n
