We are seeking a per diem Case Manager to join our team of passionate patient caregivers!

Qualifications:

Required Skills:

• Current clinical licensure strongly preferred.
• One year recent relevant experience required. Acute care setting preferred.

Responsibilities:

Works with physicians and multidisciplinary team members to develop a plan of care for assigned patients. Ensures patient is progressing towards desired outcomes by monitoring care through assessments and/or patient records. Identifies and resolves barriers that hinder effective patient care. Actively involved in discharge planning process. Integrates the hospital's mission and "Guiding Principles" into daily practice.

Posted Total Compensation The wage range for this role takes into account the wide range of factors that are considered in making compensation decisions including, but not limited to, skill sets, experience, education and training, licensure and certifications, and other business and organizational needs. It's not typical for an individual to be hired at or near the top of the range for their role and compensation decisions are dependent on the facts and circumstances of each case. A reasonable estimate of the current range is $ Minimum SalaryUSD $30.00/Hr. Maximum SalaryUSD $62.74/Hr.
