Hospice RN Case Manager — Dublin Area, On‑Call

Transitions Care in Dublin, OH is seeking an RN Case Manager to plan, organize and direct hospice care in patients\' homes and communities. The role involves travel to local homes, coordinating with an interdisciplinary team, and providing education to patients and families. You will participate in on-call duties, maintain documentation, and ensure timely adjustments to care plans as patient needs evolve.
