Supportive, innovative work environment Continuing education opportunities Responsibilities Evaluate and assess patients’ physical conditions and home environments Develop individualized treatment plans to improve mobility, strength, and function Provide hands‑on therapy, therapeutic exercise, and patient/family education Collaborate with nurses, aides, and other team members to coordinate care Supervise PTAs and participate in case conferences and in‑service education Qualifications Current CA Physical Therapist license Graduate of an accredited PT program CPR certificationite Valid driver’s license, insured and reliable vehicle Minimum 2 years PT experience (home health preferred but not required) Strong communication and interpersonal skills Knowledge of PDGM Medicare model a plus 401(k) with full match Continuing education credits Referral program Note: This job description is intended to provide a general overview of the position.
